I have concerns. Some patients may not feel able to assert themselves.

Thanks for raising this, as Moorfields doesn't seem to be communicating these changes to patients in any systematic way. I must say I share your concerns. The info in the link says PIFU is optional - I do hope that proves to be the case. And are all changes sudden? What about gradual changes which people may not register until past the point when treatment is viable?
And the new system for follow up appointments concerns me as well. Although this was introduced at the beginning of September, existing booked appointments are still being cancelled, as mine was just a few weeks ago (and it had already been postponed from earlier in the year). So until I rang the booking clerks, I had no idea when my next appt might be. I'm now told it will be reviewed next April, for a possible appt in May. So people need to be pro-active in keeping tabs on future appointments. How many will slip through the cracks? I do feel sorry for the booking clerks who are having to deal with the fallout from these changes.
Covid lockdowns shutting down clinics for months have obviously had a dire effect on the backlog of patients needing to be seen. But it's sad that four years on, the effects are still there. Has Moorfields been particularly badly affected, or are people attending other hospital eye clinics finding similar problems?
