Rejection/failure

My understanding of rejection vs failure is the former is potentially reversible and the latter is not, have I got that right?
So I'm a bit confused seeing graft failure on clinic letters but when I've asked it seems there's still potential. I'm confused.
I can see a line around the graft, it's been there a while so it's been seen, googling I think.it's the Khodadoust line. What is unclear to me is will this extend across the whole graft over time as in it turning opaque.
Any thoughts appreciated.
