OK - googling was much quicker than tackling my paperwork.
http://www.payrollgiving.co.uk
basically confirms the info I gave above. Employers with fewer than 500 employees can claim up to £500 for signing up to the scheme and the first £10 of any donations are then matched for the first 6 months. It looks as though you can fill in a donor form online on the payrollgiving site, so you don't necessarily have to go to your employer first.
Hope this helps

Ok, I've got some more details now.
Here's the simple answer:
Personally, I find the amount box a little confusing. If you write down £5, the charity will receive £5, but you will only be charged £3 or £3.90 depending on your tax bracket.
If you actually want to spend £5 and are a basic rate taxpayer, you need to write down £6.41!
Here's the simple answer:
- Download the Payroll Giving form from here
- Print the form out
- Fill in your name, address, National Insurance number and Employer's details
- Use "UK Keratoconus Self-help and support group" as the name of the charity
- Fill in the amount you'd like to donate.
- Send the form to:

FREEPOST (SWB 30457),
