On the principle that smoking in general is a bad idea, giving up or at least cutting back has to be a good idea.

So strength to your elbow.

The downside is that going "cold turkey" on a very highly addictive substance is a stressful experience at the best of times, let alone when recovering from anaesthetics/surgery and emotional stresses of transplant and looking after a new graft.
your medics may be best able ot advise on whether the stressors of smoke or withdrawal are worse.
