One thing you didn't mention in your options list was piggy backing (soft lens worn underneath an RGP or scleral lens) which seems to work very well for some people.
And while of course I agree with you that a graft should be the last option, as a graftee I have to say that some of us (about 1 in 10 with KC) do reach the point where contacts are NOT the best - acute discomfort, intolerance, useless vision - and a graft then does become the sensible option. But it's no guarantee of a lens free life afterwards! Most of us post graft still need correction with specs or contact lenses.

I had a similar immediate improvement in vision after both of my grafts. It does take some getting used to doesn't it? But wow, what a fantastic thing to experience eh? Apart from seeing that my skin wasn't flawless afterall and the house looked like it hadn't been cleaned for heaven knows how long, but we'll ignore those bits.

With regard to lenses I am an extremely tenacious person but I'd had about as much as I could stand with lenses and am very glad I had my eyes grafted. Second one was done 8 months ago and doing well. Apart from visiting local hospital where eye doctor removed some mucus from my stitches with a lump of cotton wool - and failed to numb the eye first! I am still in shock!

Update
Just to keep you up to date re: my operation.
Yesterday my eye kept on feeling like something had gone in it and then my eye started watering tonnes, this kept on happening all day so in the end I called the hospital to ask if I should be concerned, she said to come in and the doc would have a look.
If turns out that one of my stitches is leaking a tiny bit but the pressure in my eye is low. So she filled my eye with antibiotic paste and put a patch on my eye and told me to come back tomorrow.
I went in today and Mr Watts said the same that one of my stitches in leaking so he has put a contact lens bandage in my eye and has given me some more drops to put in.
Unfortunalty if it doesnt heal by next week I'll have to have an operation to put another stitch in.
The actual graft is fine though, every doctor that sees it keeps on saying its brilliant so I'll trust their judgement, and my vision is still fine.

Unless your a slow healer...the rim of the tissue should heal by monday at the latest.....I had the same issue post graft; the next day actually.....and they tried getting a bandage lens in my eye....but couldnt as I have this inherent defence mechanism which doesnt allow ppl to touch my eyes lol...so they left it...with the single stich removed...and it had healed by the 3rd day...without the bandage lens.......
